Licensed Vocational Nurse - Hospice Home Care

About the role

The Care Team, in partnership with Traditions Health, is looking for a Licensed Vocational Nurse to provide skilled nursing care in patients' homes in Crown Point, Indiana. The nurse works under the supervision of a registered nurse, coordinates care with the interdisciplinary team, patients and families and refers agencies, and meets from time to time at an alternate location. Selected candidates join The Care Team, a hospice provider, effective January 1, 2026.

Key responsibilities

• Administer skilled nursing care to clients of all ages in their place of residence, under the direction of the RN
• Participate in planning and implementing care in line with the plan of care
• Use infection control measures that protect staff and patients
• Monitor assigned cases to ensure compliance with third-party payer requirements
• Communicate effectively with patients, families, teams and other health professionals
• Identify patients' physical, psycho-social and environmental needs through documentation, records, case conferences and evaluations

Required profile

• Graduate of an accredited vocational nursing school
• One year of experience as a Licensed Vocational Nurse in a clinical care setting; home health experience preferred
• Reliable transportation, as the role is remote but occasional meetings elsewhere are probable
• Current CPR certification and Hepatitis profile
• Able to work flexible hours, travel locally, and handle prolonged standing, lifting up to 50 lbs and moving patients
