Clinical Team Manager (RN)

About the role

Traditions Health, a provider of hospice, home health and palliative care serving more than 8,500 patients across 19 US states, is looking for a Clinical Team Manager (Registered Nurse) to join its growing hospice team in Decatur, Illinois. The role ensures that home health and/or hospice services are delivered according to accepted standards of practice and company procedures, and works closely with the branch director on day-to-day office and staff management.

Key responsibilities

• Review and approve patient information submitted by licensed professionals, and review and approve orders, ensuring they are complete before approval
• Assist with patient care review meetings (case conferences and interdisciplinary team meetings) and care decisions
• Provide oversight of patient care and assist the branch director with staff supervision, including evaluations, counseling and hiring recommendations when the director is away
• Help train clinicians on documentation requirements and regulatory standards, and give orientation and in-service training to field and office staff
• Manage referrals: coordinate intake, back up the intake coordinator and patient service coordinator, and reschedule missed or declined visits
• Review patient schedules, approve schedule changes, and follow up on orders and lab reports
• Perform chart audits, quality assessments and performance improvement activities, and assist with surveys
• Take part in staff meetings, community education and administrative on-call duty
