Licensed Practical Nurse (LPN) – Prescott Community Health Center

About the role

This position provides direct patient care, including IV therapy, for assigned patients under the Licensed Practical Nurse Act and the supervision of a registered nurse. The LPN will also guide ancillary nursing staff, collaborate with the multidisciplinary team, and uphold professional standards while delivering personalized, high‑quality care.

Key responsibilities

• Utilize the nursing process to gather data, plan, implement, and evaluate patient care.
• Deliver care addressing physical, psychosocial, educational, and safety needs appropriate to patient age.
• Communicate professionally with patients, families, and team members to ensure a positive experience.
• Adapt quickly to changing unit demands and maintain flexibility throughout the shift.
• Maintain ongoing professional development, clinical knowledge, and required credentials.
• Perform additional related duties as assigned.

Required profile

• High school diploma or equivalent.
• Current Licensed Practical Nurse (LPN) license in the state of practice.
• Basic Life Support (BLS) certification from the American Heart Association.
• Excellent organizational, time‑management, and judgment skills.
• Strong interpersonal and delegation abilities.
• Ability to handle confidential information and work independently in a fast‑paced environment.
