Licensed Practical Nurse (LPN) - Hospice Home Health

About the role

Amedisys, part of the Optum family, provides home health, hospice and community-based care. As an LPN in hospice, you will work with the interdisciplinary care team to support the physical, psychosocial and emotional needs of patients and caregivers. The position is advertised for Lawrenceville, Georgia.

Key responsibilities

• Verify the plan of care with the RN before each visit
• Collaborate with the physician and RN, and notify them promptly of any change in patient condition
• Teach patients and caregivers self-care techniques
• Check vital signs and review all medications, performing medication reconciliation at each visit
• Perform specialized procedures ordered by the physician, such as a foley catheter change
• Prepare equipment and materials for treatments using aseptic technique
• Complete documentation timely and accurately

Required profile

• One year of clinical LPN experience
• Current LPN license for the state(s) of assignment
• Current CPR certification
• Valid driver's license, reliable transportation and liability insurance

What we offer

• $5,000 sign-on bonus for external candidates
• Total cash pay of $51,738 to $77,606 annually, with per-visit point pay and hourly pay
