Licensed Practical Nurse – Home Health (Part‑Time/Per Diem)

About the role

BAYADA Home Health Care seeks a compassionate Licensed Practical Nurse to provide skilled nursing and rehabilitative care to adult clients in senior living communities across Chester County. This part‑time or per‑diem position involves visiting clients in their homes and ensuring they receive the medical support needed to remain independent.

Key responsibilities

• Make scheduled home visits to clients in designated territories.
• Administer medication, perform wound care, and carry out treatments and procedures.
• Monitor client conditions, report changes to Clinical and Client Services Managers, and document doctors' orders.
• Conduct client assessments, manage cases, and coordinate care.
• Accurately record observations and interventions using a state‑of‑the‑art tablet EMR system.

Required profile

• Current Pennsylvania LPN license.
• At least one year of recent, verifiable LPN experience.
• Graduation from an accredited nursing program.
• Ability to work independently, manage time effectively, and travel to assigned cases.

Required skills

• Proficiency with electronic medical records (EMR) on tablet devices.
• Strong computer skills for documentation and communication.
