Physical Therapist – Home Health Senior Living

About the role

BAYADA Home Health Care is seeking an experienced Physical Therapist to provide home health visits in senior living communities throughout Chester County. This full‑time, part‑time or per‑diem role supports clients in remaining at home through rehabilitative care.

Key responsibilities

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests, mobility assessments, balance testing and safety evaluations.
• Develop and implement individualized care plans including therapeutic exercises and gait training.
• Educate clients and families on rehabilitative care and independent living.
• Document observations and interventions using a tablet‑based electronic medical record system.

Required profile

• Current Pennsylvania Physical Therapist license.
• Minimum one year of recent clinical experience as a licensed Physical Therapist.
• Graduation from an APTA‑approved PT program.
• Ability to work independently, strong organizational and problem‑solving skills.

Required skills

• Proficiency with electronic medical records (EMR) systems.
• Solid computer skills for tablet‑based documentation.
