Licensed Practical Nurse - Home Health, Chesapeake, VA

About the role

Commonwealth Home Care offers full-time, part-time and PRN positions for Licensed Practical Nurses (LPNs) in Chesapeake, VA. As an LPN you work under the supervision of a Registered Nurse, providing skilled nursing visits and helping create, carry out and evaluate each client's plan of care. The company is owned and operated by clinicians with over 50 years of healthcare experience.

Key responsibilities

• Provide skilled nursing care under the direct supervision of the Registered Nurse
• Assist the RN with the preparation, implementation and continuity evaluation of the client care plan
• Help perform specialized procedures and teach clients self-care techniques
• Observe, record and report clients' physical and mental condition and signs of change
• Report abnormalities in range of motion, body mechanics or body alignment to support rehabilitation
• Take part in weekly case conferences, inservices and staff meetings

Required profile

• Current LPN license in good standing with the state board of nursing
• At least one year of LPN experience; home care experience is prioritized
• Able to provide two references
• Passable OIG, Virginia State Police and national background checks, and able to verify employment eligibility
• CPR certificate, reliable transportation, valid driver's license and car insurance
