Licensed Practical Nurse – Ambulatory Primary Care

About the role

The Licensed Practical Nurse will join the Primary Care team at the Bath Health and Wellness Center, providing patient‑friendly primary and specialty services to the Akron community. Working under the direction of RNs and physicians, you will help deliver comprehensive, preventive, and chronic disease care while developing your nursing expertise within the Cleveland Clinic system.

Key responsibilities

• Provide patient care activities under the direction of an RN, physician, optometrist, or podiatrist.
• Collect and document objective and subjective patient data, complete nursing assessments, and report findings.
• Implement nursing care plans, administer medications or treatments, and monitor patient responses.
• Observe patients for adverse reactions, perform routine laboratory tests, and educate patients and families.

Required profile

• Graduate of an approved Practical Nursing program.
• Current Ohio license as a Licensed Practical Nurse (LPN).
• Basic Life Support (BLS) certification through the American Heart Association.
• One year of patient care experience preferred.

• Ohio LPN licensure.
