Licensed Clinical Social Worker - Oncology

About the role

AdventHealth in Denver is hiring a full-time, day-shift Licensed Clinical Social Worker for its Oncology program. The role promotes knowledge of oncology among patients, support partners and care providers, and provides psychosocial support and counseling.

Key responsibilities

• Meet patients and support partners after diagnosis to provide counseling and referrals tailored to their needs.
• Collaborate with the multidisciplinary team to provide ongoing support, oversee the plan of care, and take part in quality initiatives and research projects.
• Review patient information, assess patients for emotional needs and risk factors, and act as a customer service advocate.
• Serve as the primary point of contact for providers and patients needing the Oncology Licensed Social Worker (OLSW).
• Work with navigators to provide Survivorship Care Plans.
• Perform patient and family evaluations and histories, offering individual or family counseling as needed.
• Act as a resource for Infusion/Oncology practices, helping patients identify barriers to treatment and set quality-of-life goals.

Required profile

• Master's degree in Licensed Social Work (required).
• Able to communicate effectively in English, verbally and in writing (required); additional languages preferred.
• 3+ years of recent social work experience in oncology preferred.
