Intensive Support Social Worker – Adult & Aging Services

About the role

Allies4Outcomes is seeking experienced social workers to join its Adult & Aging Services Intensive Support Social Work Team. The role involves partnering with county agencies across North Carolina to deliver high‑quality, results‑driven services for vulnerable adults.

Key responsibilities

• Investigate reports of abuse, neglect, and exploitation under Adult Protective Services.
• Initiate and manage guardianship proceedings, preparing legal documentation and representing DSS in court.
• Assess eligibility for in‑home aide and support services, developing plans that promote safe, independent living.
• Evaluate placement needs for long‑term care facilities and determine eligibility for Special Assistance and Medicaid services.
• Coordinate with multidisciplinary partners—including mental health, housing, and public health—to provide crisis intervention and long‑term case planning.

Required profile

• Minimum 5 years of experience as a Social Worker III or in a supervisory role within NC DSS.
• Strong knowledge of North Carolina policy, practice standards, and quality improvement.
• Ability to work independently while leveraging technology effectively.
• Demonstrated adaptability, emotional intelligence, and change‑management skills.
• Willingness to travel across North Carolina and maintain a flexible schedule.
