Inpatient Care Manager - MSW

About the role

The Inpatient Care Manager – MSW coordinates comprehensive care for a designated patient population across the continuum of care. Working within Lehigh Valley Health Network, the manager assesses, plans, implements, and evaluates individualized care plans while ensuring safe and timely discharge planning.

Key responsibilities

• Conduct comprehensive assessments at patient entry to determine length of stay, discharge needs, and resource requirements.
• Develop and implement individualized plans of care and transition plans in collaboration with multidisciplinary teams.
• Coordinate safe and timely discharge planning, including post‑acute authorizations, transportation, homecare, and facility placements.
• Monitor patient progress through daily review, participate in multidisciplinary rounds, and resolve barriers to discharge.
• Advocate for patients and families, addressing clinical, educational, and psychosocial needs.
• Maintain knowledge of Medicare, Medicaid, and payer reimbursement methodologies.

Required profile

• Master’s Degree in Social Work.
• Less than 1 year experience in an MSW internship program.
• Ability to interact with diverse backgrounds and set priorities for care coordination.
• Proven leadership skills.

Required skills

• Familiarity with electronic health records, specifically EPIC.
• Knowledge of computer applications and analytical tools.
• Understanding of case‑management certifications (ACM or CCM) within three years.
