Inpatient Care Manager – BSN

About the role

The Inpatient Care Manager – BSN leverages nursing expertise to coordinate comprehensive care for a designated patient population across the continuum of care. This clinician‑care manager role involves assessing, planning, implementing, and evaluating individualized care plans while ensuring safe, timely discharge planning.

Key responsibilities

• Conduct comprehensive admission assessments, determine anticipated length of stay, discharge needs and resource requirements.
• Develop and implement individualized care and transition plans in collaboration with multidisciplinary teams.
• Coordinate safe, timely discharge planning, including post‑acute authorizations, transportation, home‑care and facility placements.
• Monitor patient progress daily, participate in multidisciplinary rounds, and resolve barriers to discharge.
• Educate patients and families, support self‑management, and ensure preferences are reflected throughout the care continuum.

Required profile

• Bachelor’s Degree in Nursing and active RN license in Pennsylvania (or New Jersey upon hire).
• Minimum 3 years experience in care coordination, disease management, home health, hospital or physician office setting.
• At least 1 year experience with electronic medical records.
• Proven leadership, ability to prioritize care plans, and knowledge of Medicare/Medicaid reimbursement.

Required skills

• Proficiency with EMR/EHR systems (e.g., EPIC).
• Strong computer application and analytical tool skills.
• Case management certification (ACM or CCM) within three years is preferred.
