Hospice RN Graduate

About the role

Amedisys, part of the Optum family of businesses, offers home health, hospice and community-based services. It is hiring a Hospice Registered Nurse (RN) / Licensed Graduate Nurse (GN) in Hamilton, Alabama, who works with the Clinical Manager and the interdisciplinary care team to provide, coordinate and direct hospice nursing services and case management according to each patient's plan of care, hospice regulations and organizational policies.

Key responsibilities

• Conduct comprehensive patient assessments and develop, implement and revise individualized plans of care with physicians and the interdisciplinary team.
• Manage the assigned patient caseload, including clinical episode management, care coordination and oversight of care plan changes.
• Deliver skilled hospice nursing care focused on pain management, symptom control, emotional support and end-of-life comfort for patients and families.
• Complete accurate, timely point-of-care clinical documentation in line with regulatory, payer and company standards.
• Work with interdisciplinary team members, caregivers and community resources to ensure continuity and quality of care.

Required profile

• New graduate Registered Nurse within the first year of clinical practice.
• Current, unencumbered RN license eligible for practice in the assigned state(s), including NLC eligibility where applicable.
• Current CPR certification.
• Knowledge of federal and state regulations, ACHC standards and hospice regulatory requirements.
• Valid driver's license, reliable transportation and required liability insurance.

Required skills

• Demonstrated computer, email and internet navigation skills.
• Effective communication with patients, families and caregivers of varying ages and cognitive abilities.

What we offer

• Salary of $71,640 to $107,460 per year for full-time employment, depending on factors such as local market, education and experience.
