Hospice RN Case Manager

About the role

Amedisys, part of the Optum family of businesses, is hiring a Hospice Registered Nurse as RN Case Manager in Leominster, Massachusetts. The nurse provides compassionate, patient-centered care by assessing needs, coordinating individualized care plans, managing symptoms, educating families and collaborating with the clinical team. The coverage territory is a 30-mile radius of Leominster.

Key responsibilities

• Perform the initial nursing assessment on new admissions
• Complete, coordinate and revise the total plan of care, maintaining continuity of care through collaboration with the right staff
• Communicate patient progress regularly to the clinical manager, physician and care team
• Develop, prepare and maintain accurate, timely patient care progress records in line with care center policies
• Record pain and symptom management changes and outcomes as appropriate
• Educate families on the patient's condition, needs and care
• Refer patients to other hospice disciplines when needed, or document the reason for not referring
• Perform on-call duties and other duties as assigned

Required profile

• Current RN license for the state(s) of assignment
• Current CPR certification
• Valid driver's license, reliable transportation and liability insurance
• Preferred: 1+ years of nursing experience

What we offer

• Salary of $87,560 to $131,340 per year for full-time employment, depending on factors such as local labor market, education and experience
