Hospice Nurse Practitioner (Per Diem)

About the role

BAYADA Hospice is seeking a compassionate Per Diem Nurse Practitioner to provide hospice and palliative care to clients in Tucson, Arizona. You will work as an integral member of a multidisciplinary team, delivering home‑based medical services that enable patients to remain safely at home.

Key responsibilities

• Conduct face‑to‑face hospice recertification visits in assigned geographic territories.
• Monitor client conditions, report changes to Clinical or Client Services Manager, and document physician orders.
• Perform client assessments and follow‑up care as needed.

Required profile

• Current Arizona Nurse Practitioner license.
• Minimum one year of recent, verifiable clinical (medical/surgical) experience.
• Ability to travel to client homes as assigned and strong interpersonal skills.

Required skills

• Solid computer proficiency.
• Experience with electronic medical records (EMR) preferred.
