Hospice Admission Registered Nurse

About the role

AdvisaCare, a hospice agency serving the Tri-Cities area, is looking for a Hospice Admission RN. The role provides emotional support to help patients live their final days with dignity and comfort, while conducting supervisory visits and taking part in interdisciplinary team meetings. Flexible scheduling is offered during spring and summer breaks.

Key responsibilities

• Initial and ongoing assessment of the patient's physical, functional, psychosocial and environmental needs
• Implement the individualized plan of care and its revisions as needed
• Comply with AdvisaCare Hospice policies and procedures and Medicare compliance
• Advise and educate patients and families about the disease process, self-care, end-of-life care and ethical concerns
• Supervise paraprofessionals providing services to the patient
• Prepare clinical and progress notes and take part in interdisciplinary group conferences
• Take part in the hospice performance improvement program and maintain high client, family and caregiver satisfaction
• Communicate information using current processes and technology within the organization

Required profile

• Current Michigan license in good standing
• Minimum 1 year of hospice experience
• Empathetic approach to patients and caregivers
• Ability to adapt to patients' status and needs on a full-time on-call basis
• Participates in daily field visits and weekly office meetings as indicated
