Home Health RN Preceptor – Clinical Field Educator

About the role

BAYADA Home Health Care is seeking a full‑time Registered Nurse to serve as a Clinical Field Educator. You will precept and mentor newly hired RNs and LPNs while maintaining a small personal caseload of home‑health patients.

Key responsibilities

• Provide one‑on‑one clinical training and mentorship to new RNs/LPNs during home visits and through their first year.
• Coordinate onboarding calendars, ensure attendance at required education classes, and help new clinicians build and manage their own caseloads.
• Deliver direct patient care for at least 50% of your time, including case management, medication administration, wound care, and documentation using a tablet‑based system.
• Collaborate with the RN Clinical Manager, Clinical Educator, and office team to maintain consistent training and improve clinician retention.

Required profile

• Active RN license in Arizona.
• Prior Medicare‑certified home health experience (required; HCHB preferred).
• Strong clinical judgment, communication, and organizational skills.
• Passion for clinical education, coaching, and mentorship.

Required skills

• Home health case management and admission processes.
• Medication administration, wound care, and other RN procedures.
• Use of tablet‑based point‑of‑care documentation systems.
