Home Health Physical Therapist

About the role

Amedisys, part of the Optum family, provides home health, hospice and community-based services. As a Physical Therapist you will deliver skilled physical therapy to patients in their homes, working with patients, caregivers and an interdisciplinary care team to support recovery, prevent hospitalizations and improve quality of life.

Key responsibilities

• Help patients maintain, restore and improve movement, activity and function
• Provide interventions, conduct reexaminations every 30 days at minimum, modify interventions as needed and perform discharge planning
• Perform other duties as assigned

Required profile

• Current license to practice physical therapy in the state of assignment
• Graduate of a CAPTE-accredited physical therapy program
• At least 1 year of related physical therapy experience; less than 1 year requires approval from regional clinical leaders and mentoring or a residency program
• Current CPR certification
