Home Care RN/LPN – Pediatric Home Nursing (Georgia)

About the role

H2 Homecare is seeking compassionate RN or LPN professionals to provide pediatric home care across Georgia. You will join a nurse‑owned team that offers on‑site training, flexible scheduling, and support from clinical supervisors.

Key responsibilities

• Identify and document changes in patient condition and communicate with physicians as required.
• Administer treatments and medications according to physician orders.
• Maintain accurate documentation in the Alayacare electronic medical record system.
• Clock in/out and track time using the EMR platform.
• Coordinate care with the Director of Clinical Services, Clinical Supervisor, and Case Manager.
• Request and document order modifications based on clinical judgment.

Required profile

• Current Georgia LPN or LVN license (RN preferred).
• Graduate of an accredited practical or vocational nursing program.
• At least one year of professional nursing experience; home‑care experience is a plus.
• Current CPR certification and ability to pass a background check.

Required skills

• Proficiency with the Alayacare EMR system.
