Home Care Coordinator (CNA/HCA)

About the role

Journey Nursing Services is seeking a Clinical Care Coordinator (CNA/HCA) to lead a caseload of home‑care clients while supporting field caregivers. The role blends hands‑on client care, case management, quality assurance, and team leadership.

Key responsibilities

• Manage an assigned caseload, conduct Start of Care assessments, supervisory visits, and ongoing client evaluations.
• Develop, implement, and update individualized Plans of Care and maintain accurate documentation.
• Orient, train, mentor, and evaluate caregivers, ensuring adherence to each client’s care plan.
• Coordinate staffing, anticipate coverage needs, and participate in interdisciplinary meetings.
• Serve as primary liaison among clients, families, providers, referral partners, and agency leadership.

Required profile

• Current Washington State CNA or Home Care Aide certification.
• Current CPR/BLS certification.
• Minimum two years of caregiving experience.
• Strong leadership, organizational, and communication abilities.
• Valid Washington State driver’s license, reliable transportation, and willingness to travel throughout the service area.
