Home Care Admissions Nurse – Weekend Shift with Sign‑On Bonus

About the role

The Home Care Admissions Nurse works weekends to assess patients, determine appropriate levels of care, and develop individualized care plans. This position includes a $15,000 sign‑on bonus (extra $5,000 if applied by Labor Day).

Key responsibilities

• Perform comprehensive initial and follow‑up assessments covering physical, psychological, and environmental factors.
• Collect and document data per CMS regulations with high accuracy.
• Develop individualized care plans in collaboration with physicians and the interdisciplinary team.
• Communicate significant changes in patient status promptly to the care team.
• Provide education on end‑of‑life issues, palliative care, chronic disease management, pain control, home care, hospice, and discharge planning.

Required profile

• Graduate of an NLN‑approved nursing program with a current RN license in Pennsylvania (or NJ).
• At least one year of recent direct patient care experience.
• Ability to work independently and travel to patient assignments.
• Valid driver’s license.
• Basic Life Support certification (AHA) within 30 days of hire.

Required skills

• BLS (Basic Life Support) – American Heart Association.
• Proficiency with multiple computer applications and EMR platforms.
• Knowledge of CMS Conditions of Participation and regulatory data collection.
