Health Facilities Surveillance Nurse

About the role

The Illinois Department of Public Health is seeking a highly motivated, detail‑oriented Registered Nurse to serve as a Health Facilities Surveillance Nurse. In this position you will conduct licensure, certification and complaint surveys of long‑term care facilities, prepare written documentation, and ensure compliance with state and federal regulations.

Key responsibilities

• Conduct on‑site surveys of health facilities to verify compliance with state licensure requirements and federal certification mandates.
• Prepare detailed written survey reports and documentation for each inspection.
• Travel to facilities on a routine basis to perform health and safety assessments.
• Interview residents, staff, administrators, and health‑care providers to gather information.
• Observe patient care practices and review medical records, policies, standard operating procedures, and other relevant documents.

Required profile

• Registered Nurse (RC023 bargaining unit) authorized to work full‑time in the United States.
• Highly motivated, detail‑oriented individual capable of working under general supervision.
