Geriatric Advanced Practice Provider – Full-time

About the role

The Dorothy Adler Geriatric Assessment Center is seeking a full-time Geriatric Advanced Practice Provider to join its high-volume outpatient team. You will serve as a core clinical provider, delivering comprehensive assessments and ongoing care to older adults in a collaborative, multidisciplinary environment.

Key responsibilities

• Conduct comprehensive geriatric assessments for new and follow-up patients.
• Manage complex medical, cognitive, and functional conditions independently.
• Maintain a panel of approximately 7 clinic sessions per week.
• Coordinate care with physicians, nurses, social workers, and other specialists.
• Document all encounters in the EPIC electronic medical record system.

Required profile

• Licensed Nurse Practitioner (NP) or Physician Assistant (PA) with interest or experience in geriatrics.
• Strong clinical judgment and ability to manage complex patients autonomously.
• Team-oriented mindset with a commitment to patient-centered care.

Required skills

• Proficiency with the EPIC EMR system.
