Family Medicine Primary Care Provider (MD/NP/PA)

About the role

We are seeking a dedicated Family Medicine Primary Care Provider to join our team in Bradford, VT. The role involves delivering comprehensive, patient‑centered care to individuals of all ages, with an emphasis on prevention, diagnosis, and treatment.

Key responsibilities

• Provide primary care services across the lifespan, focusing on preventive health, acute illness, and chronic disease management.
• Educate patients and families about health maintenance and disease prevention.
• Collaborate with multidisciplinary healthcare teams to coordinate care.
• Maintain accurate and up‑to‑date patient records.
• Participate in after‑hours on‑call coverage as needed.

Required profile

• Medical degree with residency training in Family Medicine or a related field.
• Board certified or board eligible in Family Medicine.
• Current, valid Vermont medical license.
• Experience in primary care preferred.
• Strong clinical judgment and interpersonal communication skills.
