About the role

The Community Health Worker Apprentice will serve as a liaison between patients, the healthcare system, and community organizations, supporting health equity and access for underserved populations.

Key responsibilities

Build community partnerships and promote health equity.
Assist patients in navigating the healthcare system and connecting them with essential resources.
Conduct outreach and education to improve community health knowledge.
Provide informal counseling, advocacy, and support for patients.
Collaborate with medical providers and community groups to coordinate care.

Required profile

High school diploma or equivalent.
Strong interpersonal and communication skills, both oral and written.
Effective time‑management, problem‑solving and de‑escalation abilities.
Ability to develop trust and work relationships with diverse individuals and professionals.
Residency in Northeast Ohio and willingness to travel locally (≈20%).
