About the role

The Community Health Worker Apprentice supports patients, the healthcare system and community organizations, gaining hands‑on experience under a seasoned CHW preceptor. This apprenticeship combines on‑site work at Lorain Family Health Surgery Center with hybrid classroom sessions.

Key responsibilities

Assist patients in navigating the healthcare system and connect them with essential community resources.
Participate in outreach activities, health education, and building partnerships with local organizations.
Support informal counseling, advocacy, and cultural competency initiatives.

Required profile

High school diploma or equivalent.
Strong time‑management, problem‑solving and communication abilities.
Ability to work collaboratively with diverse patients, families and staff.

Required skills

Experience or familiarity with Epic electronic health record system.
Bilingual ability in Spanish (preferred).
