Community Health Worker – ACWIH (Grant/Hot Springs County)

About the role

The University of Arkansas for Medical Sciences is seeking a Community Health Worker for the ACWIH program serving Grant and Hot Springs counties. This role supports postpartum women through education, home visits, and coordination with clinical teams to improve health outcomes.

Key responsibilities

• Conduct home visits to provide postpartum health education and support.
• Assist families with accessing medical, social, and community resources.
• Document patient interactions and outcomes in the health record system.
• Collaborate with OB‑GYN clinicians and public health staff to ensure continuity of care.
• Travel within Grant and Hot Springs counties to reach clients.

Required profile

• Proof of legal authority to work in the United States on the first day of employment.
• Experience or interest in community health, public health, or related fields.
• Strong communication and interpersonal skills.
• Ability to work independently and manage a caseload.
• Willingness to travel locally as needed.
