Community Health Nurse (Spanish Language)

About the role

The Multnomah County Health Department is seeking a Community Health Nurse who is fluent in Spanish to provide comprehensive health services to diverse populations. This role focuses on disease prevention, health promotion, and supporting clients in self‑care through assessment, counseling, and case management.

Key responsibilities

• Conduct comprehensive health assessments, screenings, and nursing diagnoses for individuals and groups.
• Develop, implement, and evaluate care plans for chronic conditions such as diabetes, asthma, depression, and wound management.
• Coordinate referrals and collaborate with other healthcare providers to ensure continuity of care.
• Provide health education, self‑management support, and emergency response as needed.
• Administer medications and treatments according to standing orders and established protocols.
• Document all clinical activities accurately and maintain up‑to‑date charts.
• Mentor and coach less‑experienced staff and serve as a resource for the healthcare team.

Required profile

• Current nursing licensure (RN) and ability to practice in Oregon.
• Fluency in Spanish to communicate effectively with Spanish‑speaking clients.
• Demonstrated ability to work independently and as part of an interdisciplinary team.
• Commitment to health equity, cultural humility, and community‑focused care.

Required skills
