Community Health Nurse – Public Health Services

About the role

The Department of Health is seeking a dedicated Community Health Nurse to deliver essential public health services. This full‑time, on‑site position involves direct patient care, health education, and collaboration with physicians and community agencies.

Key responsibilities

• Conduct interviews, health histories, vital signs, and nursing assessments for clients in clinic and community settings.
• Administer medications and immunizations, following ACIP recommendations and departmental protocols.
• Perform screening and laboratory tests, collect specimens, and ensure proper specimen handling.
• Document client encounters accurately, code services electronically and manually, and maintain vaccine inventory.
• Provide referrals, follow‑up care, and coordinate continuity of care with physicians and community partners.
• Educate individuals and groups on public health programs at community events.
• Manage clinical supplies, track medication inventory, and report discrepancies.
• Participate in staff development, orientation, and preceptorship of new nurses.

Required profile

• Current RN licensure and knowledge of nursing principles, anatomy, and physiology.
• Ability to assess clients independently and make sound nursing judgments.
• Experience administering medications and immunizations within scope of practice.
• Strong documentation and coding skills following departmental policies.
• Effective communication and collaboration with multidisciplinary teams.

Required skills

• Proficiency with the Verilert temperature monitoring system.
• Familiarity with the PFS (Pharmacy Fulfillment System) for medication inventory management.
