Clinical QA Nurse (RN) – Home Health & Hospice

About the role

We are seeking a Registered Nurse with extensive home health and hospice experience to own the clinical quality of documentation processed through our AI‑driven platform. In this operations‑focused role you will ensure that automated reviews of referrals, OASIS forms, certifications, physician orders and other clinical records are accurate and clinically sound.

Key responsibilities

• Audit clinical QA performance by reviewing charts processed by the automation platform for accuracy, completeness and consistency.
• Partner with engineering and product teams to refine AI decision logic, identify edge cases and validate improvements before deployment.
• Develop SOPs and QA guidelines that define how home health and hospice documentation should be reviewed across agencies.
• Work directly with client agencies to understand their documentation standards, configure automations and support implementations.
• Measure and improve quality metrics such as automation accuracy, exception rates, clinical escalations and rework.

Required profile

• Active RN license (NY or compact license preferred).
• 3+ years of clinical experience in home health and/or hospice.
• Strong knowledge of OASIS documentation and Medicare home health/hospice eligibility requirements.
• Experience reviewing physician orders, certifications, plans of care, face‑to‑face documentation and hospice election paperwork.
• Excellent attention to detail, pattern recognition and ability to work in a fast‑paced startup environment.
