Clinical Pharmacist – Full-time

About the role

The Clinical Pharmacist will join BayCare’s pharmacy team to provide patient‑focused medication management, ensuring safe and effective drug therapy across the continuum of care.

Key responsibilities

• Develop competency in clinical practice, including appropriate drug therapy and prevention/resolution of drug‑related problems.
• Evaluate patients for drug‑drug, drug‑food, drug‑lab, and drug‑disease interactions.
• Individualize drug regimens based on patient size, organ function, and disease state.
• Facilitate medication continuity at admission, transfer, and discharge.
• Review and accurately interpret medication orders.
• Dispense and compound medications in accordance with good pharmaceutical practice.
• Ensure formulary adherence, recommend cost‑effective therapies, and minimize waste.
• Support medication safety initiatives and report events through proper channels.
• Orient and train new pharmacy team members, interns, residents, and students.
• Provide direction to pharmacy technicians to maintain workflow and service standards.

Required profile

• Active pharmacist license issued by the state of practice.
• Bachelor’s degree in Pharmacy.
• No prior experience required.

Required skills

• Strong computer skills.
