Clinical Pharmacist (Milford)

About the role

The Clinical Pharmacist provides safe and appropriate pharmacy services to patients receiving home infusion care. This entry‑level position works closely with the Nursing Department to coordinate care, monitor patient progress, and educate patients, caregivers, and staff on medication use.

Key responsibilities

• Determine patient suitability for home care by collecting and reviewing medical and medication information.
• Design, recommend, monitor, and evaluate individualized pharmacy care plans for home‑care patients.
• Identify and implement modifications to prescribed medication therapy or monitoring parameters as needed.
• Gather patient‑specific data to prevent, detect, and resolve medication‑related problems.
• Establish appropriate monitoring parameters and conduct nutritional assessments.
• Adjust pharmacy care plans based on ongoing monitoring data and patient outcomes.

Required profile

• Licensed pharmacist with a valid pharmacy license.
• Entry‑level clinical pharmacy experience or strong interest in home‑care pharmacy practice.
• Ability to collaborate with nursing staff and educate patients and caregivers.
