Medical Social Worker - Hospice

About the role

Amedisys, part of the Optum family of businesses, is hiring a Medical Social Worker in Milford, Delaware, to provide psychosocial support, assessment and care coordination to patients and families receiving home-based hospice services. The role covers mostly Sussex County, with some PTO coverage in Kent County. The salary ranges from $66,400 to $99,600 annually for full-time employment, and external candidates are offered a $5,000 sign-on bonus.

Key responsibilities

• Help patients and families with health-related financial, social and emotional concerns and support effective coping skills
• Provide social work services in line with the plan of care
• Communicate changes in the patient's clinical status to the healthcare team and the care center office in a timely way
• Educate patients, families and team members on social work interventions and the treatment plan
• Perform care planning, transfers and discharge functions
• Identify risks or threats to the patient's health in their environment, act on them and inform the physician and the care team
• Connect patients with community resources and provide counseling to promote optimal outcomes

Required profile

• Masters and/or doctorate degree from a Council on Social Work Education-accredited school of social work
• Active licensure as LMSW or LCSW
• Current CPR certification
• At least 1 year of social work experience in a healthcare setting
• Meets state and federal requirements for the position
