Clinical Pharmacist

About the role

Option Care Health is seeking a Clinical Pharmacist to join its home infusion team. In this entry‑level position you will ensure safe and appropriate medication therapy for patients receiving care in their homes across the United States.

Key responsibilities

• Assess patient eligibility for home infusion services and collect all relevant clinical information.
• Develop, implement and monitor individualized pharmacy care plans for each patient.
• Recommend medication adjustments or monitoring parameters based on patient progress and clinical guidelines.
• Perform nutritional assessments and integrate findings into therapy plans.
• Coordinate medication services with the nursing department and other interdisciplinary team members.
• Educate patients, caregivers and staff on proper medication use, storage and administration.

Required profile

• Current license to practice pharmacy in the United States.
• Entry‑level clinical pharmacy experience or strong interest in home‑care pharmacy.
• Ability to analyze clinical data and make evidence‑based medication recommendations.
• Excellent communication skills for patient and team education.
