Case Worker – Family Medicine & Community Health

About the role

The University of Miami’s Department of Family Medicine and Community Health is seeking a full‑time Case Worker for its Miami campus. The role supports patients and families by coordinating services, providing counseling, and linking individuals to community resources.

Key responsibilities

• Conduct situational analyses through home and office visits, mentor clients, and deliver technical assistance as needed.
• Collaborate with service agencies, program leadership, and funding sources to ensure appropriate service delivery and resource availability.
• Develop and deliver training for family support networks and care providers on early care, education, and quality improvement.
• Maintain accurate records to ensure compliance with project guidelines and university policies.
• Assist patients with tobacco cessation, educate health‑care providers on best practices, and run support groups.
• Provide tobacco‑prevention education and support community initiatives for tobacco‑free environments and workplace wellness programs.

Required profile

• Bachelor’s degree in a relevant field.
• Minimum of one year of related experience.
• Bilingual ability preferred.
• Strong knowledge of community resources, regulations, and funding mechanisms.
