Case Manager

About the role

Day One Integrative Services is hiring a full-time Case Manager in Marion, Ohio. The role involves monitoring and assessing client progress, meeting with clients and families, providing support and referrals, and keeping accurate clinical documentation.

Key responsibilities

• Monitor and assess progress according to agency policy, regulatory requirements and best practices.
• Meet with clients individually or in groups to assess needs, collaborating with clients, families, other case managers and clinicians.
• Provide services such as crisis intervention, advocacy, assistance, financial assistance, ADL assistance, referrals and linkage to community resources; may visit, monitor and transport clients.
• Prepare clinical reports and individualized service plans using the agency's electronic clinical documentation software.
• Ensure documentation complies with contract, regulatory and agency requirements and that client records stay up to date.
• Conduct family outreach and participate in the Loved Ones Group.
• Maintain contact with probation departments on client progress and keep caseloads and productivity through regular client contact in the community and office.
• Work efficiently on a personal computer to enter, edit and retrieve data and produce reports.
• Work as a team member, taking part in all required trainings and ensuring the safety of others.

Required profile

• HS diploma or GED; an associate degree or higher in human services or criminal justice is preferred.
• CDCA licensure.
• Valid driver's license and proof of holding and maintaining any other required permits, licenses or certifications recognized by the Ohio Department of Administrative Services.
• Able to handle and solve problems involving several variables in somewhat unfamiliar settings and topics.
