Care Manager RN – Weekends (Saturday and Sunday)

About the role

AdventHealth is hiring a part-time Registered Nurse Care Manager for its Parker facility, Colorado. The schedule is every Saturday and Sunday, with 10-hour shifts each day (20 hours per week); shift times may vary. The role focuses on discharge planning and care transitions.

Key responsibilities

• Take part in multi-disciplinary rounds reviewing patient status, progression, level of care and discharge plans, and escalate care delays to leadership
• Educate patients and families about the emotional, social and financial impact of illness and advocate for their involvement in health care decisions
• Assess readmitted patients for the reasons behind their readmission
• Organize and run patient and family care conferences with the multidisciplinary team
• Document discharge plans, assessments, barriers to care and patient needs, and coordinate authorization requests with payors for post-acute care
• Review medical records and incorporate clinical, social and financial factors into the transition of care plan
• Develop discharge plans with contingency plans, arranging post-acute care and community care, and use technology to share patient information accurately

Required profile

• Registered Nurse (RN)
• Associate degree in Nursing (preferred Bachelor's degree)
• 2+ years of medical or hospital nursing experience
• Prior Care Management or Utilization Management experience preferred
• Knowledge of CMS Conditions of Participation for Discharge Planning

Pay range: $35.39 - $65.82.
