Care Manager Registered Nurse (RN)

About the role

AdventHealth is hiring a full-time Care Manager Registered Nurse (RN) in Zephyrhills, Florida, working day shifts Monday to Friday, 8am to 4:30pm. The nurse takes part in multi-disciplinary rounds and leads discharge planning for assigned patients, coordinating their transition to post-acute care and community resources.

Key responsibilities

• Take part in multi-disciplinary rounds reviewing patient status, level of care and discharge plans, and escalate care delays to leadership
• Educate patients and families on the emotional, social and financial impact of illness and advocate for their involvement in care decisions
• Assess readmitted patients for the reasons behind their readmission
• Organize and run patient and family care conferences with the multidisciplinary team
• Document discharge plans, assessments, barriers to care and patient needs, and coordinate authorization requests with payors
• Develop discharge plans with contingency plans, arranging post-acute care and community care
• Use technology to share patient information with post-acute providers so records are complete and discharge reconciliation is accurate

Required profile

• Registered Nurse (RN) license
• Associate degree in nursing (preferred bachelor's degree)
• 2+ years of medical or hospital nursing experience
• Prior care management or utilization management experience preferred
• Knowledge of community resources, post-acute care programs and CMS Conditions of Participation for discharge planning

Pay range: $33.08 - $58.04. Florida Level 2 background screening applies.
