Care Management RN (Nonexempt)

About the role

AdventHealth in Denver is hiring a non-exempt Care Management Registered Nurse (RN) on a PRN day schedule. The nurse takes part in discharge planning and care transitions, working with patients, families and the multidisciplinary team to coordinate post-acute care and community resources.

Key responsibilities

• Take part in multi-disciplinary rounds reviewing patient status, level of care and discharge plans, and escalate care delays to leadership
• Educate patients and families about the emotional, social and financial impact of illness and advocate for their involvement in care decisions
• Assess readmitted patients for the reasons behind their readmission
• Organize and facilitate patient and family care conferences with the multidisciplinary team
• Document discharge plans, assessments, barriers to care and patient/family needs, and communicate payer needs for post-acute care authorization
• Review medical records and incorporate clinical, social and financial factors into the transition of care plan
• Develop discharge plans with contingency plans, arranging post-acute care and community care
• Use technology and best practices to share patient information with post-acute providers so records are complete and discharge reconciliation is accurate

Required profile

• Registered Nurse (RN)
• Associate degree in nursing (preferred: Bachelor's degree)
• 2+ years of medical/hospital nursing experience
• Prior Care Management or Utilization Management experience preferred
• Knowledge of community resources, post-acute care programs and CMS Conditions of Participation for discharge planning

Preferred certifications: BLS, Certified Case Manager (CCM) and Accredited Case Manager (ACM). Pay range: $35.39 - $65.82.
