Care Coordinator

About the role

RECO is hiring a Care Coordinator in Delray Beach, Florida, to act as the link between admissions, clinical care and the client experience. The coordinator is the main point of contact for clients and families during treatment, coordinating scheduling and ensuring a smooth flow through every phase of care.

Key responsibilities

• Act as the primary contact for clients and families, answering day-to-day questions and concerns
• Coordinate scheduling of therapy sessions, family sessions, psychiatric appointments and medical referrals
• Work with admissions to ensure a seamless intake and orientation for new clients
• Liaise with clinicians, escalating concerns and highlighting wins
• Partner with the discharge planner on aftercare, alumni connections and continued-care referrals
• Maintain accurate, up-to-date client records in the EMR and other internal systems
• Complete additional tasks as requested by the supervisor and keep your workspace clean

Required profile

• Valid driver's license (Florida preferred)
• Ability to pass an employee drug screen
• Weekend availability
• Preferred: at least one year of experience in care coordination, behavioral health, case management or a similar client-facing role
• Preferred: CPR/NARCAN/First Aid training (provided if needed)
• Preferred: Certified Behavioral Health Technician certification
• Day shift, Monday to Friday, 8-hour shifts, in person

• Strong written and verbal communication
• Comfort with EMR systems, Google Workspace and scheduling tools
