Care Coordinator IV - Pediatric Care Management

About the role

Centene is hiring a Care Coordinator IV to lead the care coordination team that supports care management for pediatric members from birth through age five. The role ensures that providers deliver the right services and that care stays coordinated across services, acting as a liaison between care managers, providers and members. Candidates must live in North Carolina.

Key responsibilities

• Conduct assessments and screenings, develop care plans and connect members and families with resources
• Lead outreach by phone or home visits to discuss care plans, next steps, resources and ongoing education
• Coordinate care activities with healthcare and community providers and members/caregivers, accommodating changes or progress
• Support complex member and provider inquiries and escalate highly complex requests to management
• Document and maintain member records according to state and regulatory requirements
• Identify needs and refer to the Care Manager, community-based organizations and the Disease Manager
• Provide education on benefits and resources, and assist with training and development needs

Required profile

• High School Diploma or GED
• 4+ years of related experience
• Resides in North Carolina
• Compliance with standards of practice, policies and contractual and regulatory requirements
