Behavioral Health Therapist (Remote)

About the role

Mindoula Health’s Collaborative Care Program is expanding in Oregon and seeks a Behavioral Health Therapist to join its remote team. The therapist will provide evidence‑based mental health support to patients with behavioral health and chronic medical conditions, working within an integrated primary‑care model.

Key responsibilities

• Screen and assess patients for mental health and substance‑use disorders, develop treatment plans, and deliver psychotherapy via video, phone, or in‑person visits.
• Support psychotropic medication management, monitor treatment adherence, side effects, and clinical outcomes.
• Track patient progress in the electronic health record (EHR), document encounters, and coordinate care with medical providers and psychiatric consultants.
• Participate in weekly caseload consultations, contribute to process improvement, and meet monthly performance metrics.

Required profile

• Master’s degree in Social Work, Psychology, Counseling, Rehabilitation, or related field.
• 3‑5+ years of behavioral health clinical experience, including case management and community care coordination.
• Active independent clinical license in Oregon (LCSW, LPC, LMFT, or Licensed Psychologist).
• High‑speed home internet and ability to work Monday‑Friday, 8:00 AM‑4:30 PM PT.
