Advanced Practice Provider – Outpatient Kidney Medicine (West Region)

About the role

The Cleveland Clinic Department of Kidney Medicine seeks an Advanced Practice Provider (NP, PA, or CNS) to deliver outpatient kidney care across the West Region, including clinic and hemodialysis settings.

Key responsibilities

• Evaluate, diagnose, and manage adult patients with hypertension, chronic kidney disease, kidney stones, and electrolyte disorders.
• Conduct thorough histories, physical exams, order and interpret labs, imaging, and other diagnostic tests.
• Prescribe and monitor medications within state and institutional guidelines.
• Educate patients on disease processes, treatments, and medication side effects.
• Participate in multidisciplinary team rounds and coordinate care with physicians, nurses, and other specialists.
• Assist with quality monitoring, discharge planning, and regulatory compliance.
• May serve as preceptor, conduct research, and support professional development of staff.

Required profile

• Nationally certified Advanced Practice Provider (NP, PA-C, or CNS) with appropriate state licensure.
• Graduate of an accredited APRN or Physician Assistant program; MSN required within 3 years for APRNs.
• Current Basic Life Support certification; ACLS, PALS, or NRP as required by work setting.
• Ability to work collaboratively in a multidisciplinary team.

Required skills

• Clinical management of hypertension, chronic kidney disease, and related renal conditions.
• Interpretation of laboratory, radiologic, and cardiographic data.
• Prescribing authority and medication monitoring.
• Proficiency in BLS; ACLS, PALS, or NRP as applicable.
