Advanced Practice Provider (NP/PA/CNS) – Chronic Care Clinic I East Side PRN

About the role

The Cleveland Clinic Health System seeks a nationally certified Advanced Practice Provider to deliver primary and specialty care in the Chronic Care Clinic on the East Side. This PRN position works collaboratively with a multidisciplinary team to promote health, prevent disease, and manage acute and chronic conditions.

Key responsibilities

• Obtain comprehensive medical histories, perform physical examinations, and order appropriate laboratory, radiology, or special tests to develop diagnoses and treatment plans.
• Interpret routine diagnostic procedures, prescribe and monitor medications within licensure, and educate patients about their conditions and therapies.
• Participate in medical team rounds, coordinate referrals, ensure continuity of care, and support discharge planning and regulatory compliance.
• Contribute to quality monitoring, evidence‑based practice translation, research activities, and professional development of the care team.

Required profile

• Graduate of an accredited APRN program (MSN required within 3 years of hire) or an A.R.C. approved Physician Assistant program with a master’s degree.
• Current state licensure as a Nurse Practitioner, Physician Assistant, or Clinical Nurse Specialist, plus national certification in the respective role.
• Basic Life Support (BLS) certification; Advanced Cardiac Life Support (ACLS) or Pediatric Advanced Life Support (PALS); Neonatal Resuscitation Program (NRP) or Advanced Trauma Life Support (ATLS) as required by the work setting.
• Credentialed and privileged by the Medical Executive Committee and the Governing Body of the applicable hospitals.
