Advanced Practice Provider – Heart Failure (Outpatient)

About the role

The Cleveland Clinic Heart Vascular and Thoracic Institute seeks an experienced Advanced Practice Provider (NP/PA) to join its Heart Failure outpatient team. This full‑time position focuses on delivering high‑quality cardiology care in an outpatient setting.

Key responsibilities

• Obtain comprehensive histories, perform physical exams, order and interpret labs, imaging, and cardiographic studies to develop diagnoses and treatment plans.
• Collaborate with physicians, nurses, and multidisciplinary team members during patient rounds and care coordination.
• Prescribe and monitor medications within state and institutional guidelines, educate patients on disease management, and ensure continuity of care.
• Participate in quality monitoring, discharge planning, and compliance with regulatory standards.

Required profile

• Current NP and RN licensure in the applicable state(s) with national NP certification.
• Acute Care Certified or Adult/Adult‑Gero certification; cardiology experience is mandatory.
• MSN degree from an accredited APRN program (or obtained within three years of hire).
• Strong analytical, decision‑making, and communication skills; ability to work independently in a fast‑paced environment.

Required skills

• Basic Life Support (BLS) certification.
• Advanced Cardiac Life Support (ACLS) or Pediatric Advanced Life Support (PALS) certification.
• Proficiency with electronic medical record systems.
• Credentialing and privileging by the Medical Executive Committee.
