Advanced Practice Provider (Acute Care NP/PA) – Heart Failure

About the role

The Cleveland Clinic Heart Vascular and Thoracic Institute seeks an experienced Acute Care Nurse Practitioner or Physician Assistant to join its Heart Failure team. The role provides inpatient consult coverage and will transition to primary service on a Heart Failure intermediate‑care hemodynamics unit.

Key responsibilities

• Provide inpatient Heart Failure consults across cardiology, medicine and specialty services.
• Rotate between a 10‑bed Hemodynamics Unit (Swan‑Ganz guided therapy) and the HF inpatient consult service.
• Develop expertise in advanced heart failure management alongside a leading transplant team.
• Conduct comprehensive histories, physical exams, order and interpret labs, imaging and cardiographic studies.
• Prescribe and monitor medications within state and Cleveland Clinic policies.
• Educate patients on disease, treatment plans and medication side‑effects.
• Participate in multidisciplinary rounds, discharge planning and quality monitoring.
• Serve as a clinical preceptor, support professional development and may assist with research.

Required profile

• Minimum 3 years of Cardiology or Critical Care experience.
• Acute Care Certified Nurse Practitioner or PA with a graduate degree from an accredited program.
• Current state licensure as RN/NP or PA, including DEA registration where required.
• Basic Life Support certification; Advanced Cardiac Life Support or Pediatric Advanced Life Support as applicable.
• Credentialed and privileged by the Medical Executive Committee.

• Interpretation of laboratory, radiologic and cardiographic data.
• Medication prescribing and monitoring within regulated scope.
