Admissions Registered Nurse, RN – Home Health Visits

About the role

BAYADA Home Health Care is seeking a compassionate Admissions Registered Nurse (RN) to provide skilled nursing care to patients throughout Montgomery County, Pennsylvania. The role focuses on delivering personalized care in patients' homes, from hospital discharge through ongoing home health support.

Key responsibilities

• Provide 1:1 skilled nursing, medication administration, wound care, and comfort treatments according to individualized care plans.
• Conduct client assessments, educate patients and families, and monitor condition changes.
• Oversee wound care consultations and ensure proper documentation.

Required profile

• Active, current RN license in Pennsylvania.
• Minimum one year of nursing experience, with prior home care experience required.
• Ability to travel to assigned cases and maintain a calm, reassuring presence.
