ACT Team Counselor

About the role

The State of Georgia’s Pathways Center is seeking a licensed ACT Team Counselor to join its Assertive Community Treatment (ACT) team. This role provides intensive, community‑based mental health services 24/7, helping individuals with serious mental illness and co‑occurring substance use disorders achieve stability and independence.

Key responsibilities

• Deliver professional therapeutic services to clients with mental illness and/or addictive disease.
• Participate in daily multidisciplinary treatment team meetings.
• Conduct comprehensive face‑to‑face assessments and develop individualized treatment plans.
• Write timely progress notes, risk assessments, and crisis intervention documentation.
• Facilitate admissions, maintain communication with referral sources, families, and unit staff.
• Provide crisis intervention and may work non‑traditional hours as program needs dictate.
• Perform case management duties to ensure continuity of care across providers and community resources.
• Develop and maintain collaborative relationships with community service providers.

Required profile

• Associate or full licensure as LAPC, LMSW, LPC, or LCSW.
• Master’s degree in social work or counseling from an accredited institution.
• At least two years of experience in a related social work or counseling role.
