About the role

Lehigh Valley Health Network is seeking a Registered Nurse to join its Home Care Admissions team on weekends. The role involves determining the appropriate level of care, completing assessments, and creating individualized care plans while delivering patient‑centered education.

Key responsibilities

• Perform comprehensive initial and follow‑up assessments of patients, including physical, psychological, and environmental factors.
• Collect and submit CMS‑required data with high accuracy and formulate individualized plans of care based on physician orders and interdisciplinary collaboration.
• Communicate significant changes in patient status promptly to physicians and the care team.
• Provide education to patients, families, and staff on topics such as palliative care, chronic disease management, pain control, and discharge planning.

Required profile

• Graduation from an NLN‑approved nursing program and current RN license in Pennsylvania (or NJ) upon hire.
• At least one year of recent direct patient care experience, preferably in home health or hospice.
• Basic Life Support (BLS) certification and a valid driver’s license.
• Ability to work independently, manage time effectively, and travel to patient assignments.

Required skills

• Basic Life Support (BLS) – American Heart Association.
• Proficiency with multiple computer applications and electronic medical record (EMR) platforms.
• Knowledge of CMS data collection requirements and Conditions of Participation.
