About the role
The Transplant Coordinator will manage the full spectrum of care for solid organ transplant patients, working closely with a multidisciplinary team that includes nephrologists, dialysis centers, and other healthcare providers. This position ensures seamless coordination from admission through post‑transplant follow‑up within a nationally recognized health network.
Key responsibilities
- Coordinate admission of potential donors and recipients for surgery and manage inpatient transplant care.
- Work with the Organ Procurement Organization, physicians, and hospital staff to arrange organ procurement and transplantation.
- Evaluate transplant candidates, schedule pre‑transplant evaluations, and oversee outpatient pre‑transplant care.
- Facilitate post‑transplant medication protocols, including immunosuppression management.
- Maintain 24‑hour on‑call availability for the transplant program and ensure all pre‑operative studies are completed and communicated.
Required profile
- Bachelor’s Degree in Nursing.
- Minimum 4 years of nursing experience in medical, surgical, or dialysis settings.
- Current RN license (or PA license) in the State of Pennsylvania.
- Demonstrated knowledge of current transplant‑related medical information.
- Preferred: Master’s Degree in Nursing.
